You have been caring for someone with a serious, incurable illness. You don't need to be known to the Inpatient Unit to make a referral.
Who are the services for?
Our services are available if you have been bereaved by the death of a loved one from a palliative illness.
You don’t need to be known to the Inpatient Unit to make a referral.
Who can ask for the services?
Referrals are accepted from
- Family member / Carer
- General Practitioners (GP’s),
- Acute and Community Specialist Palliative Care Teams,
- District Nursing teams and Palliative Patient Key Workers.
- Any professional working with family
How to access the services?
A community outreach form must be filled out. You can find links to the forms below.
