Who can ask for the services?
Referrals are accepted from
- Family member / Carer
- General Practitioners (GP’s),
- Acute and Community Specialist Palliative Care Teams,
- District Nursing teams and Palliative Patient Key Workers.
- Any professional working with family
How to access the services
A community outreach form must be filled out. You can complete this by following the links below.
