The person you meet could be a Social Worker, Nurse, or Chaplain.
Social Worker
Our social workers will support you and give you information. They help you as you care for someone with a serious illness. They help you talk about serious illnesses in the family. They can help to talk about your loved one’s illness, care needs, and future plans. They can help you know what to tell your children. They can help you get other services, like financial support.
Chaplain
Our chaplains listen to you about your life and family and what matters to you. They listen to you share your fears and worries. Spiritual support is for everyone.
Nursing
Our nurses help you manage your loved one’s symptoms. They listen to your worries about caring for a loved one at home. They advise on pain medicine and managing tricky symptoms. We support you and your family by listening and offering comfort during tough times. They can direct you to other services.
Who can access 1-2-1 Services?
You are caring for someone with a serious, incurable illness. You don't need to be known to the Inpatient Unit to make a referral.
