Admissions and Referrals
Referrals are accepted from hospital consultants, general practitioners, acute and community specialist palliative care teams and palliative key workers.
Inpatient unit
Reason for referral
Referrals to hospice care should only be considered when the person has palliative diagnosis or condition.
Patient are referred to the inpatient unit for several reasons:
- Complex symptom management
- End of life care
- Rehabilitation following treatment
- Respite due to complexity of condition
Who can ask for the services
Referrals are accepted from
- General practitioners (GPs)
- Acute and community specialist palliative care teams
- Hospital consultants
- District nursing teams and palliative patient key workers
How to access the services
All referrals must be completed by.

Community service
Reason for referral
- Anyone living with a palliative diagnosis or condition
- A family member or carer of someone with a palliative diagnosis or condition
- You don’t to have been known to Inpatient Unit to make a referral

- Family member / carer
- General practitioners (GPs), acute and community specialist palliative care teams, hospital consultants, district nursing teams and palliative patient key workers.
How to get the services
Any referral must have completed a Hospice Community Outreach Service referral form accessed below.
